Root folder name must equal zip name (minus .zip); zip must contain exactly one root folder with exactly two subfolders 'reports' and 'META-INF' (case-sensitive). (CSSF guide 3.2; EBA plain-csv slides; CBI guide 3.3; AFM 103 note)
What triggers it
The single most common structural rejection (103 / ICTO010-012 / CBI 'inner directory' / DNB 'META-INF en REPORTS mappen ontbreken'). Also triggered by zipping the parent directory, by 'Reports' with a capital R, or by extra files at root.
How to fix it
Zip the folder itself; verify with an archive listing: <name>.zip -> <name>/META-INF/reportPackage.json and <name>/reports/....
